Mission

Our mission is to advance access to serious illness care through education, research, advocacy, and workforce development while honoring our legacy organizations. Our commitment is to ensure that all people affected by serious illness can access high-quality care and support. This means empowering individuals to make informed choices and connections to resources that strengthen the continuity of care through the end of life.

President

Joseph Matty is an expert in nonprofit leadership, philanthropy, marketing, board management, and advocacy. For over 20 years, he has led strategic planning, increased revenues, enhanced the visibility and reputations of organizations, overseen capital campaigns, and educated nonprofit leaders. He is a founding member of a national consulting firm for nonprofit growth. He has served on the boards of higher education, healthcare, and civic and professional association organizations including serving as CEO, COO, and President of foundations and associations. Matty also served six years in the U.S. Air Force.

Services Offered

The HAP Foundation focuses on educating communities and professionals on hospice and palliative care; engaging in community-based research to better understand and impact disparities in hospice, palliative care, and grief support; increasing the CHW workforce to strengthen and share serious illness resources and education in the community; and advancing policies that assist individuals during the end of life. Our CHW team offers free culturally affirming education on serious illness and grief while connecting the community to resources. Our Missing Pieces program supports families following the death of a child by assisting with the navigation of grief resources.

Current Projects

In August 2024, our CHWs will begin hosting support groups for caregivers of patients with dementia. Our HAP Center for Learning offers a Department of Labor-registered CHW Apprenticeship program, designed to provide the skills and instruction to become a CHW and support clients or patients who are seriously ill. In partnership with AmeriCorps Seniors Workforce Program, we provide adults 55+ in five Illinois counties, with the opportunity to develop skills to be a CHW. Organizational impact to the community

In 2023, our CHW team reached over 17,000 people with serious illness-related education with 99% affirming that it was helpful. End-of-life and grief can create crises for individuals and communities. Our CHWs are trusted messengers who walk with community members, offering education and support. Our Missing Pieces team served 173 families in 2023, holding space for their grief experiences and connecting them to grief and other support services.

Funding Sources

The HAP Foundation is funded through a combination of public and private grants, generous philanthropic donors, including planned gifts and endowments, corporate sponsorships, and revenues from consulting and association management services.
