This course is designed to provide community health workers (CHWs) with an overview on a disease which encompasses a range of conditions that affect the heart and its functioning. The CHWs will learn how to explore the collection of issues that can affect the heart which are differences in health across different geographic, racial, ethnic and socioeconomic groups. CHWs will learn how to support a client with this type of disease and be able to provide proper referrals and resources.
